Use your best judgement, would be the only advice I can truly give. I think if I were there and in your shoes, and moving out of the state wasn't an option, I'd mark " yes " -- but I don't know the laws and ramifications in NY. Any possibility you could just move out of NY? Also how old is your child. Vaccines are only mandated for schools, so if your child is not school-age yet, maybe it's less of an issue to say " yes " and not be investigated? Anyone else from NY here that can offer some advice? I also felt that there should have been an area where you could place some words of your own.
