I KNOW he is definitely benefiting neurologically from the hbot, but I'm afraid that when opponents read your letter, they will lean on the carbon monoxide poisoning in that case. A team of AGH physicians this morning briefed reporters on the tratment they have been giving Randal McCloy, 26, since he arrived at the Pittsburgh hospital yesterday evening. Mr. McCloy was the only one of 13 miners retrieved alive from the West Virginia mine, 40 hours after an explosion Monday. Doctors said they have twice given Mr. McCloy treatments in a hyperbaric chamber, which provides pure oxygen within a pressurized chamber. They said he tolerated the treatments well and they intend to give him several more. But they cautioned that the benefits of the treatment will probably be long-term, rather than immediate. His carbon monoxide levels were already reduced at West Virginia hospitals. Doctors said the chamber is an adjunct to the main treatment, which is keeping his physical condition stable so his organs and brain can recover. They said he still has multi-organ failure and is in critical condition. AGH doctors said it may be weeks before the extent of Mr. McCloy's neurological problems are known. Brain scans found swelling in the back of the brain in regions that control sensation and possibly vision. But until a series of scans can be taken and compared, doctors won't know if the swelling is increasing, decreasing or staying the same, said Dr. Valeriano. Scans also showed three, tiny, pinpoint bleeding areas toward the front of the brain. But until doctors can talk to or at least observe Mr. McCloy out of a coma they won't know how much damage there is. Dr. said Mr. McCloy is being kept in a medically induced coma to help rest his brain and keep him from removing a breathing tube. When drug levels fall, Mr. McCloy bites down on the tube and his eyelids flicker, but doctors said that appears to be reflexive and they can't read too much into that. They won't know until the drugs are withdrawn whether he will remain in a physical coma. The first lady of West Virginia, Gayle Manchin, said six or eight of Mr. McCloy's relatives are with his wife, , in Pittsburgh, during the treatment. She said McCloy is realistic but optimistic. AGH doctors said they would consult with doctors from Ruby Memorial at West Virginia University this afternoon. Mr. McCloy could return there over the weekend following several more hyperbaric treatments. Dr. s Zikos said those treatments are designed to prevent long-term neuropsychiatric problems that sometimes have been shown to arise months after a brain injury. Doctors today repeatedly said that Mr. McCloy's youth and good health likely contributed significantly to his survival. After Mr. McCloy was transferred to Pittsburgh yesterday, Dr. said, " I am not expecting Mr. McCloy to jump up at the end of this treatment. Complete recovery will take a long time and we plan to keep him here until we have exhausted the benefits [of the treatment]. " Hyperbaric chambers surround patients with pressurized oxygen. They have been in use for decades, generally for treating people with acute carbon monoxide poisoning or for treating deep sea divers suffering from decompression sickness, known as " the bends. " Other uses have been proposed for hyperbaric therapy, such as speeding healing and helping the body fight infections. Allegheny General's hyperbaric chamber is a clear plastic cylinder about 3 feet in diameter and long enough to accommodate an adult. Patients are slid into place through a hatch at one end of the cylinder. Mr. McCloy spent more than 40 hours 260 feet underground, and was likely deprived of oxygen for a period of time no one has yet determined. A series of tests in town indicated " mild ischemia, " meaning inadequate oxygen. It's not clear whether hyperbaric treatment is of help when the brain has been deprived of oxygen. " There are no big studies looking at that, " said Dr. O'Toole, director of hyperbaric medicine at the University of Pittsburgh Medical Center. " It would be my guess that they're doing it to pull out all the stops. " Dr. O'Toole added, " There's probably little downside to trying it. If it helps, that's great. " When taken to WVU, Mr. McCloy had a collapsed lung that doctors first assumed was a result of the blast inside the mine. But Dr. Lawrence said they found no other injuries suggesting Mr. McCloy received the force of the blast. He speculated that Mr. McCloy's collapsed lung could have been the result of lying on his left side for the extended period of time he was unconscious inside the mine. His lung had reinflated and was functioning properly yesterday, the doctors said. Lying in the same position for a long time also led to muscle breakdown and the release of proteins that injured Mr. McCloy's kidneys. Dehydration may have contributed to the problems. (Although, I am sure the HBOT will be billed under a CO poisoning dx because that is reimburseable.) Thought it might help. The articles here are also the same in the local papers, mixed about CO and brain injury. I think they are going to finalize it as CO because, and only because it is a covered indication with insurance. I can only suggest as I am doing, hang on to the news paper articles stating brain injury and get as many as you can. If every one here hangs on to these and passes them along to each other on this list and other lists for the same cause, perhaps some day it will help in our cause to include oxygen deprivation. I am just hoping for a recovery for him, they should not have waited and I believe they received enough calls etc. to have him moved to a HBOT facility which was a good move. Guest guest Posted January 9, 2006 Report Share Posted January 9, 2006 Where CO poisoning is concerned it is important to remember that the treatment is not isolated to the brain and therefore not strictly considered a BRAIN INJURY THERAPY. Is BRAIN INJURY a result of CO poisoning?....possibly and even probably..... But not strictly. Even though most on this list are focused on the brain injuries, remember this; as much as we want this tragedy to have a silver lining somewhere and bring some positive attention to the benefits of hyperbarics it isn't entirely the same thing. Would I treat anyone with a CO poisoning with HBOT ? Absolutely, but with a specific protocol which is different from the Brain injury protocol. Following the initial CO treatment the protocol would then be altered to accommodate and focus on the possible remaining brain damage. Although somewhat similar we are still talking apples and artichokes here. It will not be appropriate to use this example of HBOT when referencing most of the cases involving our children. Some may be similar, but most will not be. We need to be careful and precise when we present our case to the powers that be so that we can be irrefutable in our proof and of stable basis for our demands. Guest guest Posted January 9, 2006 Report Share Posted January 9, 2006 On this; it has been shown that although the common diagnostic for CO poisoning is the carboxy-hemoglobin levels...there is not always a direct correlation. If the tissue was affected for long enough and then the CO level reduced where the blood level has improved (or washed) the tissue may still have residual damage and therefore benefit at a cellular level. This topic has been an argument for years in the HBOT field with how and how many HBOT does it take and is it beneficial. Some of the major insurance groups are now altering their coverage of CO poisoning and calling it EXPERIMENTAL again to establish a basis for refusal of payment. Anyone in the hospitals seen this happen yet ? Funny how they can call it " indicated " and then reverse their position to " experimental " !?! Seems to me as though another revolution is needed. To me the idea of an insurance company deciding or determining what medical treatment |I can have is outrageous. It is a fact that HBOT for MS and CP is not (yet) available on the UK National Health service but this has nothing to do with insurance companies. That has not prevented the establishment of privately run charities which are fully insured to provide HBOT for people with CP and MS. I was chatting to a guy from Portland Oregon at a Christmas party and it turned to " what if " scenarios. He said " if there had not been a revolution we would have a national health service and cops without guns. " An interesting point of view I thought but then it was Christmas and it had been a long evening with a lot of strong Punch.
