I posted an answer to this question in the FAQ's section, as I discussed this issue quite fully with Dr. Kolb when Daryl was fighting to find a surgeon here in Las Vegas who would use drains on her. It seems that it shouldn't be such an uphill battle to find a doctor who will do what is best for US, not what is expedient for them, but it seems the use of drains is thought of as unnecessary by many doctors. However, Dr. Kolb wrote several interesting comments to me, and I have included them in my answer to the question about drains in the FAQ section. Here it is:

According to Dr. Pierre Blais, the space between the capsule and the implants is contaminated by micro- organisms, silicone oils, degradation products and gel impurities and constitutes a major problem. An explant without the use of drains may not allow fluid with bacteria, fungus and silicone to leave the body. According to Plastic Surgeon Dr. Kolb, over 50% of her silicone and over 80% of her saline capsular cultures are positive for bacteria and, in her words, "any surgeon who is thinking straight would want to put in drains. " Using drains can be viewed as a large aid in the effort to heal, allowing the body to easily evacuate contaminants from the body, rather than leaving them for the immune system to fight and try to eliminate through slower channels.
