The etiology is inflammation due to bacteria but of course the mouth is full of numerous strains of bacteria. It is almost always asymptomatic and usually extraction is the course of treatment. Most of the time it is not readily visible in the mouth until it gets pretty severe and is usually diagnosed by x-rays. I have seen it on children even. It is pretty rare though.
