It is pretty accurate and basically only limited by the talent of the user. One can achieve good successes the first time you use it (I did) but true diagnostic skill only comes with practice and high level technique requires the level of practice that a concert violinist must perform. Interestingly, dowsing works equally for skeptics or believers. The basic pendulum consists of a small weight of 1/4-1/2 oz, say a piece of jewelry, suspended on a string that is 6-12 " long. There are millions of variations and one should try making them as well as trying ones made for sale. Every " new age " shop sells them. Sensitive ones react almost instantly and powerfully and then some are dull and nonresponsive. It's good to get one that matches your own temperment. One way to use it is to hold it still over the object in question and then observe which way the pendulum begins to oscillate. You can find ley lines or energy patterns such as auras by swinging it back-and-forth in front of you and walking slowly forward, stopping when the swinging converts to side-to-side at it hits the energy pattern. The choice of tool is subjective and one shouldn't rule out the other dowsing tools such as bent coathangers, hazelwood sticks and other objects that are used. Other forms include Tarot decks, the I Ching (my other favorite), Oiwiji boards and even crystal balls. Now computer programs are available that measure energy patterns and then diagnose or treat. There are dozens of good books on dowsing and pendula in bookstores, particularly new age bookstores and I recommend people who want to use one get a good book or two and begin practicing. Dowsing should never be used to substitute or short-cut research or knowledge-gathering say, for example, selecting the best liver detox plan, herb or flushing technique but once one has researched and studied all the techniques then the dowsing tool can be used to match the best techniques for yourself or a particular patient. The pendulum is actually searching and seeking your memorized repetoire for what you know to be the truth. I hope this helps and also stimulates you to give it a try!
