Dear Annie,

A scratch from a dog is a roundabout way to get Brucellosis but it's

probably possible. There are other zoonoses (infections from animals) out there

that cause swollen lymph nodes and fevers. If you have lymph nodes swollen in

your lungs, you might have gotten the primary infection by breathing it in. Your

best bet would be to go to an infectious disease guy, but a pulmonologist may

also know how to diagnose infectious disease too. Don't pulmonologists treat

tuberculosis? Possibly you have an atypical mycobacterium. Do you have a

cough?

What you have is called an FUO, a Fever if Unknown Origin. Here's part of an

Fever of Unknown Origin

Fever of unknown origin (FUO) was defined in 1961 by sdorf and Beeson as

the following: (1) a temperature greater than 38.3°C (101°F) on several

occasions, (2) more than 3 weeks' duration of illness, and (3) failure to reach

a diagnosis despite one week of inpatient investigation.1

This article provides a review of the etiologies of FUO and a rational approach

to investigating a patient with this interesting condition.

Pathophysiology

FUOs are caused by infections (30-40%), neoplasms (20-30%), collagen vascular

diseases (10-20%), and numerous miscellaneous diseases (15-20%). The literature

also reveals that between 5 and 15% of FUO cases defy diagnosis, despite

exhaustive studies.

Variations in FUO, as found in the literature, reflect the populations and

periods studied. In children, infections are the most common cause of FUO,

whereas neoplasms and connective-tissue disorders are more common in elderly

persons. FUOs that persist for more than one year are less likely to be caused

by infections and neoplasms and much more likely to be due to granulomatous

diseases (the most common cause in these cases).

Diagnostic advances continuously modify the spectrum of FUO-causing diseases;

for example, serologic tests have reduced the importance of HIV and numerous

rheumatic diseases (eg, systemic lupus erythematosus [sLE], juvenile rheumatoid

arthritis , rheumatoid arthritis [RA]) as causes of FUO. Modern imaging

techniques (eg, ultrasonography, CT scanning, MRI) enable early detection of

abscesses and solid tumors that were once difficult to diagnose.

Patients with undiagnosed FUO (5-15% of cases) generally have a benign long-term

course, especially when the fever is not accompanied by substantial weight loss

or other signs of a serious underlying disease. These findings suggest that the

underlying cause is one of the more serious diseases that initially manifest as

FUOs. Such underlying diseases are usually diagnosed after an intensive and

rational diagnostic evaluation.

Age

More than 30% of FUO cases in persons older than 50 years are related to

connective-tissue disorders and vasculitic disorders. Giant cell arteritis (GCA)

and polymyalgia rheumatica (PMR) are the two principal connective-tissue

etiologies, accounting for 50% of the cases.

I know it goes way to quickly. I can't believe he has been straight

for 2years already.

We started packing last week and can hopefully get moved before thanksgiving. We

are trying for a place in golden by heritage square. If we get it you and Liv

will have to stop by since we will be closer. It also has a pool so you guys

could come over in the summer to swim since that is he favorite thing to do.

IT HAS NEARLY BEEN 4 YEARS! I MIGHT HAVE PUNCHED IT UP A FEW MONTHS-- AND I CANT

TAKE LYRICA IT AFFECT BI POLAR AND MY BI POLR IS MANAGABLE SO GREATLY NOW, AFTER

I WORKED HARD WITH THE DEPAKOTE AND THERAPAY FOR 21 YEARS.

sO MEDS THAT AFFECT IT RE OFF LIMITS AND THERE RE OTHERS THAT INTERFERE TOO--

LIKE CYMBALTA AND NEUROTYN-- THE SSRI'S ETC.

THIS SI JOINT IS BEHIND THE HIP, LIKE ON THE BUTTOCK AND IT WILL BE 40 MGS OF A

STERIOD LIKE MEDICATION, HE SAID.

 I LOVE THE LIFT A STERIOD GIVES ESPECIALLY IN THE MOOD-- I FEL SO TIRED ALL THE

TIME -- CHRONIC FATIGUE AND I PUSH MYSELF TO DO THINGS EACH DAY, SWIM IN PT, GO

OUT, SING AND WRITE-- BUT HOUSEWORK IS NEARLY OUT AND I HAD TO HIRE A

HOUSEKEEPER WHO COMES 2 TIMES A MONTH.

I STIULL CLIMB STAIRS.

I HAVE NERVE PAIN TOO, FROM INFLAMED NERVES-- AND NERVE ENDINGS THAT ARE

INFLAMED. I NEVER SAW ANY BRUISING FROM THE HEART CATH-- BUT THE INNER GROIN

Dear Ann,

Has it already been 4 years with the steroids? I remember when you first started

with that " overdose " one doctor prescribed. My sister was also prescribed a

large dose of steroids for her back pain, but I told her what happened to you.

She's had some mental health issues (PTSD also plus other stuff) and when I told

her about your reaction, she decided to tell her psychiatrist, who said no way

should she take that much steroid. She went to another doctor and started with

the epidurals.  Too bad the first epidural didn't help at all.

Ann, did you have a lot of bruising from the Cath? Pain from early May!  They

must have done a lousy job. The inner thigh/groin is such a sensitive area to

begin with.  Does the area spasm? Is there a lump where they put the catheter?

My sister's pain doctor gave her a prescription for Lyrica. He says her

widespread pain is probably fibromyalgia.  My sister has to check that med out

with her psychiatrist too. She's on psychoactive ones that may preclude it.

Does the SI joint cause problems with the hip? or is the arthritic hip causing

problems in the SI joint?  Do they give steroid injections in the hip?

The body is wonderfully clever in all it's interconnections, but once one thing

goes wrong, watch out.

He is now 6yrs old and is doing great. If you haven't already you can read his story on the ISOP website under congenital scoliosis stories. He was decompressed at 18months old and wore three casts for a total of 6months and then a brace for 1yr. So far his surgery has been successful and there has been no signs of the Chiari at this time. We always just monitor him since there is no cure for Chiari just treatments. He does not have a syrinx but there are a few websites I would recommend. One is a support group for Chiari and syringomyelia called wacmaparents the other is www.asap.org this site has a lot of good info and Dr's that are very good in the treatment.
