Hi Jim, it's Judy g., I know in California, you call the MDA and tell them you need to 'Register' to be seen in Clinic. They will send you a simple form that your doc needs to just put your diagnosis, why you need to be seen (he can just put progressive weakness, just about anything is fine) and then he signs it. You mail it in, call and make sure they received it and they will call you for an appointment - does that help? I hope so Peace & Love Judy g. P.S. If your neurologist or specialist gives you a problem, go to your primary care physician - they KNOW they don't KNOW much and are usually happy to send one off to the MDA Clinic. P.S.S.
