We started making a medical Info sheet for everyone in the family several years ago. It saved us many hours of grief with my grandfather- he had a stroke at the barbershop- the medical info sheet let them know who to contact and what his medications were- So we make sure everyone has their sheet filled out and have a copy in their purse, wallet or the glove box of the car. And my mom and I carry forms in our purses for each member of the family- just in case. A blank of the form is below- everyone is welcome to copy and fill in their own information- it is also great cause I just update and print when we go to the dr.- saves me and the dr lots of time! I no longer have to take my meds with me or remember each med every time. I just give them a copy of the printout. if ya can think of anything else we should add to the form- let me know! Karolyn HERE IS THE FORM: Name: 1. Personal information: Address: In case of emergency notify: 1. 2. Physicians: 1. 2. Please Note: I have a living will and medical power of attorney on file. 2. Insurance information: Primary: Secondary: 3. Medication information ( in chart format) TAKE NAME AMOUNT ACTUAL DOSE RX/Dr Delete the following- but this is what the categories mean: * Take= (Yes, Not right now or as needed) * Name= name of RX * Amount = dose amount- mg, cc * Dose= how much in a day * RX/Dr= who wrote the rx 4. Medical information Known Allergies: Have the following medical conditions: Previous surgeries: -------Original Message------- Subject: Karolyn and All If you need to call 911 (God forbid) this is a very, very useful tool for the paramedics and the ER staff. In a time of crisis, people forget vital things. I'd like to encourage everyone to do this. I cannot tell you how many times I was able to save someone or buy them time by this kind of information being given to me when I walked in the door. Keep a copy at home and carry one with you! Marsha Karolyn and All If you need to call 911 (God forbid) this is a very, very useful tool for the paramedics and the ER staff. In a time of crisis, people forget vital things. I thought I order directly from the company but I can't find their website. I got them for $10 plus shipping. I have allergies to metals. We go for our shopping Sat but I might stop by and ask the pharmacy for the bracelet. We don't get Dierbergs here in central Texas.
