I always kept a milege log with the dates times and got a doctors note from the doctor stating I was there that day. Now if you are taking a taxi, I would get a receipt from the driver every trip and a doctors note from your doctor. Then you can claim it under medical expensives if you itemize your taxes. Keep in mind though you have to have accumulated a certain amount of medical bills. I tend to file medical bills every two years as I do not accumulate enough annually.
