Both can occur in just chronic hepatitis and are not unequivocal evidence of cirrhosis. Perhaps you should have a good ultrasound meanwhile to look for splenic and portal vein size and collaterals and to look for the liver surface and texture, that can tell a lot sometimes, if the radiologist is clever. Fluvoxamine maleate( Luvox) is a selective serotonin (5-HT) reuptake inhibitor (SSRI) , bound to plasma albumin , it is extensively metabolized by the liver and its mean plasma half-life at steady state after multiple oral doses of 100 mg/day in healthy, young volunteers is about 15 hours. Thus in those with liver disease the drug elimination will decrease by about 30% thus potentiating its effects on the brain. It should be slowly titrated in patients with liver dysfunction during the initiation of treatment. Further , elevation of AST/ALT has also been a reported adverse effect so unless it is absolutely necessary it should not be used in those with cirrhosis of liver.
