Guest guest Posted December 19, 2008 Report Share Posted December 19, 2008 Care pathway for depression and anxiety INTEGRATED CARE PATHWAY FOR ANXIETY AND DEPRESSION Purpose: This Integrated Care Pathway is for patients at Park Avenue Medical Centre presenting with, or suspected of suffering from depression and/or anxiety. It is intended to guide assessment and intervention ensuring that a patient's journey is negotiated, managed and agreed. As mental health care provision is limited there needs to be clarity regarding inputs and interventions required and how they will be delivered. Interventions are based upon evidence and best practice. It is essential that the expectations of the individual patient are addressed as part of the overall care pathway. Objectives: Provide a clear pathway to guide decision making and interventions. Provide evidence based interventions for patients. Provide a rationale for specific interventions depending on where the patient is on their journey to recovery. Provide a system for recognizing deviations to normal practice.
