Apart from Spinal Cord Injuries, I wonder if it can be used for MS and other nervous system damaging diseases? Friday (I seem to recall maybe reading something about it on here, but not entirely sure Dear Uncle Spine: What is the latest on 4-AP, the next-big-thing drug we've been hearing about forever? Norvell 4-aminopyridine, a.k.a. 4-AP, is a nerve signal booster. It works by jacking up conduction in nerve fibers that are whole but don't work well because of faulty myelin insulation. About one in three SCI people will improve with 4-AP, but it's not possible to predict who will benefit or what effect the drug will have on those who do. Some get movement, some get erections, some get pain relief. Sometimes the results are quite dramatic. Acorda Therapeutics, a New York company, has completed a Phase II clinical trial testing a time-release dosage of 4-AP on people with spinal cord injuries. Results are not available yet but the word on the gimp grapevine is that the doses were too light and therefore the drama was light. Still, the company hopes to get on with a much larger Phase III trial this year. A recent report from the Long Beach VA fuels enthusiasm for 4-AP. Physician Jack Segal studied 21 patients, all at least two years post- injury. On average, they regained at least 1 to 1.5 neurological levels of function after three months of getting the drug. " There were significant changes in patients' ability to carry out activities of daily living, " he says. The drug also improved pulmonary function by 30 percent to 40 percent, a big deal for quads and vent-users. Well, you say, I want my 4-AP. Of course you do. Ask your doctor to prescribe it (it's legal, but totally " off-label " since the drug's not approved for any use other than bird repellent). There are many pharmacies in the U.S. that will formulate 4-AP for you. It's usually less than $50 a month and you will have to play with the dose. But don't play too much.
