Dear All,

I just got to answsering these questions in an email fromone of the

members fromNoveber 14th! I am sorry I have not had time. We are

going through many changes at work and a move. Got very behind. I

hope this helps someone.

The member gave me a question and possible answers. I hav NO clue

where the questions came from other than the member her/his self.

I am only posting those that are not math related on this post. In

addition I am not posting the two questions that do not make sense to

me. I will recontact the member for more details.

1. ***Which of the following systems keeps a running inventory

balance? Physical, annual, Biannual, Perpetual

Answer is: Perpetual

2. ***When a tech delivers a narcotic to a patient care unit and

cannot

find a nurse to sign for the delivery, what does he/she do with the

meds?

Return meds to Rx, NEVER leave a controlled substance with someone

who does not have the authority to accept/sign for it or in an in-box

where some one pick it up and leave with it.

3. ***How is the DEA number made up? I hear there is a formula to

ensure it is valid.

YES there is a formula. Unfortunately I do not post this so that

itdoes not getinthe 'wrong hands'. However I am sure it is ont he

net. I just won't be a party to posting it and perpetuating its

illegal use. ( I am not sayig you would do this, but I do not know

who would surf my site and not have the safety of the public in

mind).

4. ***The hospitals morphine injection inventory is depleted on Friday

and the wholesaler does not deliver again until Monday morn, What

does the pharmacists do?

Fill out a DEA Form 222, borrow from a near hospital follow state

laws and regs and DOT for transportation to the pharmacy.

5. ***Preparing and packaging Hydrocortisone ointment for use by more

than one patient is referred to as:

Unit Dose? Parenteral Admixture? Unit of Uses? Bulk Compounding?

The FDA Classification for a specific drug will be on ITS Package

Insert.

The actual descriptions of all of the pregnancy risk categories are

on the FDA site/lit.

SPF = Security Prescription Form in CA is good for 6 months.

I am not sure if all states have the SAME law.

16. ***What is a lamiar flowhood?

A device or piece of equipment used to provide a sterile envioronment

in order to compound medications, especially IV's there are Verticle

and horizontal laminar flow hoods. The Flow of the air current is

controlled and it provides filtering of the air up to 0.3 micron in a

HEPA FILTER = High Efficiency Particulate Air Filter. Recalll that

the smallest bacteria is 0.2 microns so some may pass and contaminate

the environment/IV/compounding.

17. ***What kind of equipment is a hepa filter used in?

18. ***What is bioavailability?

The amount of drug that is available to be used by the body for

therapeutic purposes after absorption and protein binding is

completed.

19. ***What is a highly light sensitive drug?

Photosensitive drugs are sensitive to light and should be stored

under specific temperatures, lighting and packaging.

20 ***A client calls the pharmacy freaking out b/c his inhaler has

been recalled. What should be done?

Throw away and get another refill? Send it back to the manufacturer?

Call Manufacturer to see if it is the one recalled?

CALMLY Ask the patient to check the lot number while they are on the

phone with you, if it is the recalled lot number, bring in the

inhaler, that the pharmacy will replace it with another inhaler at no

charge. If he still is concerned or freaking out pass the phone to

the pharmacist who may call attention to the fact that he sounds fine

most likely is not affected by the drug at this time; if necessary

get to hospital or doctor ASAP. Ask them to have a relative or friend

pick up a new inhaler. Document the phone call.

21. ***Mrs. Brown was treated for hypertension- What kind of drug did

the DR. put her on?Dieuretic or Antiemetic?

Diuretic to reduce body fluid which will reduce blood volume which in

turn reduces blood pressure.

22. ***What is the Orange Book used for?

Orange book is for rating the trade vs generic formula for generic

equivalency.

23.***Which book is most like the manufacturers package insert that

tells about the med? Orange Bk? PDR?

PDR is more like the package insert. But it is not considered

reliable as USP references such as F & C

24. ***Pharmacy just received a bunch of boxes that are still in

storage, when you go get them they are out of date, should we: Send

them back to manufacturer? Throw away and order more? Report it to

FDA?

Send back to mfg or wholesaler (who ever sent them) and ask for in

date product, do not pay for them. If paid ask for exchange.

25. ***Drugs on the code cart are used for what?

Code blue emergency

26. ***Someone's LDL was 286 is that high or low?

High

27. ***What is the general pharmacy procedure in case of a class I

recall?

Call all patients that a have a script of drugs that had the lot

number of the recalled med, tell them it has been recalled, bring it

in return it to the pharmacy ASAP and the pharmacy will issue another

prescription when they bring it in. Remove ALL product from the shelf

with the recalled lot number. Send a letter to all that can not be

reached by phone.

28. ***If a drug expires on 12/08, what is the date you can dispense

until?

12/31/08 last day of the month
