Dr. Bihari has most of his patients on 4.5 mg and about a quarter on 3 mg. If you stay on that dose for more than six months or so and don't see any improvement, you can talk to your doctor about petitioning for a dosage increase. You can see the chart for recommended dosages according to weight and IgE at www.xolair.com. I weigh 130(ish) and my last recorded IgE was 700+. I get 375mg every two weeks.
