2.)SPOC: This is a service provided by the makers of Xolair, to help

patients who encounter problems getting Xolair. This may include

prior authorization and recertification, as well as appeals if their

insurance provider denies Xolair. SPOC also provides co-pay

assistance for people having problems affording Xolair. Individuals

looking for assistance will need to fill out the Patient

Authorization and Notice of Release of Information (PAN) Form. This

is a form that a patient signs in order for Genentech programs to

have access to the patient's protected health information for the

purpose of obtaining reimbursement or appeals assistance. The form is

available online.
