Hi Beverly,

I had both my knees replaced in Aug. 2010 and found out recently that I needed

to have periodontal work done. It made me rather nervous also. I am allergic

to penicillin so the prescribed clindamycin HCL for me. (two 300 MG capsules an

hour before dental work) I have used it now about 5 times with no problems.

Perhaps they can try another medication instead of the amoxicillin for you.

Amoxicillin was the only one I was told to take.

I have never, ever, been prescribed any drugs before any fillings or teeth

cleaning at a dentist. The only drug I've ever been given before a procedure

was something to knock me out so they they could remove one of my wisdom

teeth. I was given some pain relief following that though. But antibiotics?

Never. And I've never had a problem. You don't get given antibiotics in the

UK unless they are absolutely necessary, as they will become less effective

if used too often. I am really surprised to hear that they are given before

dental procedures as a 'just in case'!

procedure. The fake knees, hips etc are more liable to infection.

I was given a Z-Pak before dental work in Mexico. Consists of 4 pills.

Dentist said works about the same as amoxicillin. That was for dental surgery.

My present local dentist said do what your OS says, mine said antibiotics

forever.

I got my fill of water exercises during my 7 week stay near Palm Springs.

The exercise instructor would say don't do this exercises if you had TKR.

Most of the time they were the exercises my PT had me do. Mostly the twisting

exercises. So I did the exercises. I went into their Walmart to buy some

noodles for the pool. Temperature was 90 degrees out. Walmart said we don't

have them till summer. I asked her what she calls summer and she said 120

degrees.

time as having a kneeor hip iop, or similar?

Lin, any time you have a false joint in situ, be it one month or ten years

you should take an antibiotic even for a clean and scale.

Hi Lin, What we are saying is that you have to take an antibiotic every time you

go to the dentist for anything for the rest of your life! Even when you have a

cleaning, the debris that is cleaned out can be swallowed, and that debris is

usually full of bacteria. Ditto for a colonoscopy. You have to take

antibiotics before that procedure also. I really scares me that some of my

friends in the UK are not following this procedure. Is the NHS really that

bad??? It certainly raises all sorts of questions in my mind. I would question

your OS about this immediately!

Joan, whenever I travel overseas and even when I went to UK last year I

always take a prescription of antibiotics in case something untoward happens

I don't take them just for the sake of it, I came home again with them.

I remember once when we were there the time before last and I was going to

run out of my pain medication, I went with my script to my mother in laws GP

and he refused to give them to me, so my mother had to post them from

Australia to me in UK in hopes of getting them in time, it worked out okay

they weren't stopped by customs thank goodness.

Just unnecessary hassle in my opinion,

Yes well, we should here also Barb, as I think Australia has the highest

rate of skin melanoma in the world, I don't get mine checked I must admit.

But I don't go out in the sun either. Not a sun baker.

Hi Lin and everyone

I suppose its about balancing the risk of an infection getting into your

implant against the overuse of antibiotics.

I've never heard of anyone treated in the UK -NHS or private being told to

take antibiotics before the dentists but pretty well all the people I know

from America are. So I guess that we (I'm UK) see the risk as lower then

they do in the USA.

Be interesting to find out more. I'll ask a friendly surgeon

Thanks - but I have had a clean since the op, no problems. I will check

this with my GP though, just in case. This issue hadn't even been raised

with me while in hospital, and it isn't mentioned in the leaflets they give

you.

I talked to a friendly UK surgeon about this and he said that he tells

patients to use antibiotics for any MAJOR dental work. He queried if the

emphais in USA on antibiotics for any dental work might be driven by a

fear of litigation.

I've asked another surgeon - but it's Sunday so guess he's in his garden

chilling out

Hi Lin, What we are saying is that you have to take an antibiotic every time

you

go to the dentist for anything for the rest of your life! Even when you

have a

cleaning, the debris that is cleaned out can be swallowed, and that debris

is

usually full of bacteria. Ditto for a colonoscopy. You have to take

antibiotics before that procedure also. I really scares me that some of my

friends in the UK are not following this procedure. Is the NHS really that

bad??? It certainly raises all sorts of questions in my mind. I would

question

your OS about this immediately!

Hi , Yes, I carry a Z-Pac wherever I go. Actually, now that you mention it,

I should get a new one as the one I have is probably out of date. Yes, getting

prescriptions filled in another country is not easy. Although we have used the

NHS doctors for some things, I always bring a full supply of my meds with me,

and have a friend in NYC who ccan mail me more if need be. I was a Girl Scout.

Joan, whenever I travel overseas and even when I went to UK last year I

always take a prescription of antibiotics in case something untoward happens

I don't take them just for the sake of it, I came home again with them.

I remember once when we were there the time before last and I was going to

run out of my pain medication, I went with my script to my mother in laws GP

and he refused to give them to me, so my mother had to post them from

Australia to me in UK in hopes of getting them in time, it worked out okay

they weren't stopped by customs thank goodness.

Just unnecessary hassle in my opinion,

I agree Joan - information is a good thing! As for weather, the last few

days here have been like high summer - most unusual. But it's back to normal

on Tuesday according to the forecasts.

to Lin - Perhaps your dentist is not updated on your condition. In the US, the

American Academy of Orthopedic Surgeons studied the evidence base and published

a statement mentioning joint replacement with their recommendation for

prophylactic antibiotics for patients on their website <http://bit.ly/fsy9Lx>

and the American Dental Association posted it on their website as well.

I wondered about how to search the NHS because you are in UK, and found a

somewhat different statement titled " Recommendations for antibiotic prophylaxis

for dental procedures in patients with joint replacements " from

Newcastele-on-Tyne Hospital letterhead with National Health Service imprimateur

As for antibiotics, I was told one year after TKR by my surgeon, 2 years by my

dentist, and the rest of my life by various people. Which one is right? I

recently did the amoxicillin 2000 mg. before dental work. My poop came out as

water for 5 days afterwards. I corrected this imbalance with some candida

natural compound, lots of yogurt, and sacchromydes boullardi, a specific

probotic. I am okay now. I don't look forward to this procedure for the rest of

my life. So what is the real time frame? Devora

I find all this very interesting! I had my TKR done at Cleveland Clinic, and was

told nothing about antibiotics " just in case " . I even asked, because I have

lymphedema and was told that it wasn't necessary unless I get sick. I do have

antibiotics in the cabinet, but they are there because I tend to get frequent

bladder infections.

This philosophy seems wrong to me because of all the antibiotic resistant

diseases coming about now. Also, I see a lot of people who are suddenly allergic

to certain antibiotics due to " overdose " . This happened to me. One day I was

taking penicillin for an infection (had been taking it for about a week) and the

next day I was itchy & swollen!

So I'm sticking with what my doctor told me (or didn't tell me!) and not using

antibiotics like aspirin!

Hi Pat,

My surgery was done at Southpointe Hospital which is also under the

Cleveland Clinic group. I think it depends on what your doctor feels is

write.

My dentist of many years said the American Association of Orthopedic Surgery

came out with a bulletin in 2009. It basically says you need them every time

you get your teeth cleaned or have dental work. Here is the link in case

I have an appointment on Thursday for a six month check up so I will have to

ask about other antibiotics.
