Polyps aren't strictly speaking a cause of ctoma but may be something 'heralds' it (as the article puts it). Phil Having Ear Polyps Removed Ear polyps are round, red lumps of tissue that bleed readily, even with gentle manipulation. They are a sign of chronic inflammation, and they usually arise from the tissue that lines the middle ear (mucosa) through a hole in the eardrum. Polyps can also arise from the outer surface of the eardrum or from the ear canal skin, but the middle ear is the most common source. Since your ear, nose and throat doctor (ENT) has discussed mastoidectomy as an option (your second choice), I assume that your polyp does, indeed, arise from the middle ear. Polyps are significant for two reasons. First, they say something about the severity and duration of middle ear inflammation. You have chronic otitis media, which is defined as a nonhealing hole in the eardrum, and is often associated with chronic middle ear inflammation. In addition to the perforated eardrum, chronic otitis media can cause hearing loss, dizziness, ear pain and chronic drainage from the ear. Polyps are also significant because they can herald the presence of cholesteatoma, a nasty complication of chronic otitis media. Cholesteatoma is best defined as "skin growing where skin ought not to grow." In this instance, skin may be growing in your middle ear space. This is a problem because skin tends to produce more skin. The outer layers of skin die and flake off. Usually these flakes are so tiny that you don't notice them, but occasionally you may be conscious of this process. (Two examples are dandruff and sunburn peeling.) When skin grows in the middle ear, the dead skin cells may not be able to exit. What results is a growing ball of skin -- not a true tumor, by the way, but it may as well be a tumor. This skin ball (cholesteatoma) can grow to enormous size, destroying everything in its path. It may even erode the base of the skull and punch its way into the brain. Cholesteatomas are slow-growing, however, so they are often compared to a time bomb with a very long fuse. Trouble is, you don't know how long this fuse is. The diagnosis of cholesteatoma is sometimes obvious. Dead skin ("epithelial debris") has a characteristic appearance. However, if all the ENT sees on examination is a polyp, he won't know whether a cholesteatoma is present. That's where a CT comes in. A CT scan can help in the diagnosis of cholesteatoma. Unfortunately, it can be difficult, if not impossible, to differentiate a small cholesteatoma from chronic middle ear inflammation on CT. Polyps can certainly be removed via the ear canal (your first option), but if your surgeon finds a large cholesteatoma lurking behind the polyp, you may need a more extensive operation (your second option). I don't have a crystal ball, and neither does your surgeon. Even if you do not have a cholesteatoma, it is very possible that the polyp is just the tip of the iceberg: The mucosa throughout your middle ear space (which includes the mucosa within the mastoid, the bone immediately behind and below your ear canal) may be extraordinarily inflamed. If this is the case, removal of the polyp will have little impact on your condition -- once again, you would need the more extensive operation. So, unfortunately, I cannot tell you if the first procedure is going to work. Neither can your ENT. You may, indeed, need the second procedure. Should you get another opinion? That was treated with a 'wick' and antibiotic drops. We were psyched when we went back and the polyp was gone, we thought we had our problem whipped. However, the ENT noticed a 'pearl' lurking behind my sons eardrum, which led us to where we are now. My father in law gets bad ones in his throat and had to go off bread, wine, and take a bunch of supplements. I don't know for sure which ones they are. I'd see a naturopath about it. Quoting " J. He's in his 40's and only has trouble with it when the wind blows from the SW. He usually ends up with a sinus infection when his gets irritated. I'll suggest that he see a naturopath about it. It has got worse the past few days and in my forehead to sothink it's actually a shot at sinusitis. Hope this doesn't bring them on again.I forever live in fear of those polyps.Muxinex sounds good - Is this American? Can it be obtained in the UK? Yea my discharge is mainly watery but have getting thicker stuff out too. I have updosed my nasonex from the 2 sprays once a day to twice a day. So this may reduce them a bit. Any know know of anything else that can relieve the intense pressure? The NeilMed rinse or some other sinus rinse should really help. The watery discharge could also be “leftovers†from having irrigated. I know before I had my ethmoidectomy, saline would get stuck up there and then run out at random. Now I actually hang my head upside down, either by doing yoga or simply touching my toes, to help drain my sinuses after irrigation. And the other benefit of this is I can actually touch the floor now! I’ve been using an antihistamine nasal spray when I get really stuffy, that has also really helped. My allergist suggested that. I only use it when I need to. Decongestants are an option but I avoid them. They dry me out so bad they make me nauseous and I personally don’t think drying out the sinuses is very good for them (based on how stuffy I can get in the winter when it is dry!). I have taken sinus sachets in a drink that contain this along with paracetamol and something else for a cough?! lol. That has hit the spot within 30 mins but needs taking every 4-6 hrs. It was a bit better yesterday until the eveningand then it hit me full force again and had to sleep sitting up as I couldn’t breath lying down. It may or may not cause problems later on but I used it with no prob for 15 years there was then a manufacturing problem so none was available and polyps came back within a month. I use 2 sprays in each nostril if I'm REALLY bad, 1 spray in each works well for maintanence. When all is going well, I will alternate 1 spray in 1 nostril and the next day, 1 spray in the other one. I have used this for several years now, and it has worked well for me. My ENT said a nipple stretched over the mouthpiece works well, too. I haven't tried that. Hope this helps. It appears I'm the queen of polyp production, with no diagnosis of what allergies cause it in spite of loads of allergy tests. (It doesn't appear to be classic samters, and oh do I feel for you sufferers.) Last spring an ENT doc suggested that I put a liquid suspension of Budesonide the last bit of my daily saline irrigation. This isn't the usual nose spray but is stronger dose, intended for nebulizer use for asthma. He said that he couldn't guarantee that it would give me smell/taste back, but would keep me out of operating room for longer. Oh. my. goodness. In three days, I actually was smelling. Breathing better. Felt great. I used it for the 3 months he said, then was off for 2 months and at the end I was stuffier, losing smell/taste, presumably polyps had grown again. I just started developing the asthma and asa intolerance but I have been suffering from polyps and sinus infections for the last 7 years. I couldn't tell you how many times I have been prescribed pregnisone over that time but I can tell you that I have developed cataracts as a result. Yea me! I have been told that the cataracts are a result of the nasal steroids coupled with the pregnisone. I know as samters sufferers, we all have to pick our battles but I believe we are the best equipped to do so with as much knowledge as possible. Wishing you the best, Rob. The best way to determine food problems is by doing an elimination diet and keeping a food diary. I use 2 sprays in each nostril if I'm REALLY bad, 1 spray in each works well for maintanence. When all is going well, I will alternate 1 spray in 1 nostril and the next day, 1 spray in the other one. What I didn’t realize when I was using it was that it can also affect mood, at least it affected my mood, very negatively. But so do other forms of steroids. I’d just be on the watch for this as well as issues others have mentioned.And I can’t emphasize enough =-- by far the most proven method for reducing or eliminating polyps is aspirin desensitization (right after surgery to clean the polyps out- if surgery is necessary).
