can someone please help me with my mgb OHIP application, please & thank you

If there is anyone who has any suggestion or helpful tips with thise I

would really appreciate it. So far I have not had any luck with getting

much response to my email. Could someone tell me where to start with

this as so far anyone that I ask says that they cannot help. There has

to be someone who can help me please. For many of you who have already

had the sugery will remember the stress you went through with getting

everything organised for sending your package to OHIP. I am not trying

to be pushy or rude I just need anyones help please.

I am almost ready to fax in my application to OHIP. For the Section

Part 4B For treatment not performed in Ontario (include names and

physicians and/ or health facilities contracted in Ontario to determine

whether treatment is performed) I was wondering if anyone would be

able to provide me with a list of physicians and or health facilities

I would really appreciate any help I could get
