I applied for it in MS and it went to the LA office in New Orleans to get done. I was not asked about anything relating vaccination. Guest guest Posted September 6, 2006 Report Share Posted September 6, 2006 I have all of the information I need to get my passport and all you need is two pictures that are passport size , you can get them at Wal-mart and then you need a copy of your original birth certificate that is stamped from the place where you were born. Town clerk's office has tehm. There is no place in it that asks for any other information. They cost about $100.00 and they take about 6-8 weeks to get back.
