I think it is really up to you, it is mainly a cosmetic thing-- not like the back a misshapen head doesn't affect the brain. However, it depends on the degree of misshapen as to things that can be affected. Our daughter's head was not flat but lopsided, for lack of a better descriptor. We decided to do it because wearing store bought sun glasses would be impossible for her because her ears were in different locations, helmets would never have fit her properly (and won't fit a flat head properly either), and since we had a girl, we didn't want to have regrets when she was a teenager and asked us why we didn't fix it when we could. I am trained to properly fit bike helmets on kids and I can tell you that I see tons of flat headed children out there, so I think it is just a parental preference. I have a friend whose son is 4 and really needed a helmet when he was younger and I would hate to look back when Evan is older and regret not doing it.
