My alt med doc prescribes the Cortef twice > daily, the last dose being at 2 pm., so I haven't had much trouble > sleeping. I'm currently increasing to 15 mg. am and 10 mg. at 2 (was on > 10 am and 5 at 2). Hi, Dahlia, What I've learned about Cortef or Isocort is that the goal is to " recreate " the natural cortisol curve of a healthy individual, with highest cortisol in the AM and decreasing from there. As mentioned above, the Natural Thyroid Hormones Adrenals group does have good info. There is also a dosing schedule at http://www.stopthethyroidmadness.com/adrenal-info/how-to-treat And according to Peatfield, the thyroid meds should not even be attempted to be increased until the adrenals are stabilized. More info at http://featherstone.bravehost.com/thyroid/peatfieldadrenal.html Peatfield also says that some people may require more than 20mg of cortef. Pulse rates of 41-55 is pretty serious business. I wish you the best! Warmly,
