Bad reaction or quality concerns your first stop is your own doctor who

can file a medical report with the FDA.But you must ask your Dr to do

it.

Want to submit a consumer alert

go to FDA's site Med watch at www.fda.gav/MedWatch.

You will be asked to describe the medication, the adverse event and

some brief medical history.Your identity is not requested.But if you

supply an e-mail address you will receive confirmation that your report

was received.

You can also speak to an FDA representative at 1-888-INFO-FDA

Contacting the manufacture or distributor The FDA requires prescription

and over-the-counter drugs to list contact information on the packaging

Return the medication to the pharmacist Use one you know and trust
