The effect of exercise is such that most experienced athletes will not use statins for fear of predisposition to muscle side effects of all kinds. The severity of the muscle side effects from statins vary widely from relatively benign CPK elevation without symptoms, to muscle tenderness, aching, soreness and pain, and rarely, even to death from rhabdomyolysis. These complaints range from transient global amnesia, to aggravation of pre-existing senility to forgetfulness, disorientation and confusion.The mechanism of this side effect is clearly rooted in the biosynthesis of cholesterol and fundamental to neurophysiologic mechanisms. Only in the past several years have we learned the importance of cholesterol in brain function. When statin drug manufacturers make the claim of reduction of cholesterol synthesis, it is based upon inhibition of the mevalonate pathway, shared by the other major elements such as CoQ10 and dolichols.
