(V.P.); l.korkina@...(L.K.).AbstractWhilst facing a worldwide fast increase of food and environmentalallergies, the medical community is also confronted with anotherinhomogeneous group of environment-associated disablingconditions, including multiple chemical sensitivity (MCS),fibromyalgia, chronic fatigue syndrome, electric hypersensitivity,amalgam disease and others.These share the features of poly-symptomatic multi-organcutaneous and systemic manifestations, with postulatedinherited/acquired impaired metabolism of chemical/physical/nutritional xenobiotics, triggering adverse reactions atexposure levels far below toxicologically-relevant values, often inthe absence of clear-cut allergologic and/or immunologicinvolvement.Due to the lack of proven pathogenic mechanisms generatingmeasurable disease biomarkers, these environmentalhypersensitivities are generally ignored by sanitary and socialsystems, as psychogenic or "medically unexplained symptoms".The uncontrolled application of diagnostic and treatment protocolsnot corresponding to acceptable levels of validation, safety, andclinical efficacy, to a steadily increasing number of patientsdemanding assistance, occurs in many countries in the absenceof evidence-based guidelines.Here we revise available information supporting the organic natureof these clinical conditions.Following intense research on gene polymorphisms of phase I/IIdetoxification enzyme genes, so far statistically inconclusive,epigenetic and metabolic factors are under investigation, inparticular free radical/antioxidant homeostasis disturbances.The finding of relevant alterations of catalase, glutathione-transferase and peroxidase detoxifying activities significantlycorrelating with clinical manifestations of MCS, has recentlyregistered some progress towards the identification of reliablebiomarkers of disease onset, progression, and treatmentoutcomes.PMID: 21845158 [PubMed - in process]
