Butcertain practical things should be addressed more keenly.1) Lack of diplomas/certificate courses/fellowships in General Medicine andGeneral Surgery- We have a "all or none" phenomenon in these branches. An MDMedicine can medicolegally handle any type of medical case, but at the same timean MBBS has so many limitations (medicolegally and otherwise)that he isreduced to a mere spectator (a cough and cold GP!).MCI should introduce shortterm courses for MBBS doctors to upgrade their knowledge and practice in theirfields of interest short of MD; for eg. One year certificate course in EmergencyMedicine, Diabetology etc.This will bridge the gap between MD and MBBS.2) The insulting tag of "Non-clinical branch" attached to half the medicalspecialities- It's absolutely non-sense. It is the primal egos of certainso-called "clinicians" (physicians and surgeons)that makes them derive wickedpleasure by degrading some fellow post-graduate doctors in fields like pathologyand radiology by calling them "non-clinical". Any sane person would know what'sthe truth. MCI should remove the overemphasis on non-medical aspects of suchsubjects,for eg. Too much of physics in Radiology. Let us leave thetechnicalities to the engineers and focus more on clinically relevant things forRadiologists. Introduce a 6 months to one year compulsary post in Medicine orSurgery as a part of 3 year curriculum for MD/DNB Radiology. Similar steps canbe taken for Microbiology, where the MD students should be given clinicalpostings and later on allow them a clinical fellowship like Infectious Diseasesafter MD. Similarly MD subjects like Pathology,Physiology,PSM,Forensic Medicineetc. should be brought in mainstream clinical practice, so that students dontshy away from these important branches as they fear becoming "non-clinical".Each and every doctor (post-graduate or not) should develop the basic clinicalskills to handle day to day cases. I remember a story of quarrel between different body parts for who is superior. A consensus was reached that stomach was a lazy organ which did nothing except eating. So, they starved it. Ultimately, the whole body became weak. The other body parts realized their mistake and since then all the body parts were considered equal. In a nutshell, each and every subject is equal.
