Shirley: You need to call Dr. Chock (or the doctor on call) - 425-385-2263.

The receptionists can get you a code for the Realize website on Monday. The

post-op nurses are suppose to give it to you so it may be in your stuff, but

giving people information when they are drugged frequently means it gets

lost. Just call and get another one tomorrow.
