I am doing research in Pharmacovigilance for which I need survey questionnaires

to be filled by the practising Doctors. I am enclosing a study questionnaire

developed for doctors. I shall be grateful if you could kindly get the attached

questionnare filled by you. If possible please get printouts of questionnaires

and get the same filled up by your fellow coloeagues in your area/institute. As

many as possible from your area and then post them back to me on my office

address. I shall be highly grateful to you.
