Hello mam,

Thank you for your valuable input . Your concern is genuine, but here we are discussing QOL in healthcare. That includes both, use of QOL measures in clinical research(which is what you are concerned with) as well as making use of research findings to take decisions in health care administration. Both aspects have been covered in earlier posts.

Using QOL measures in clinical practice ensures that treatment and evaluations focus on the patient rather than the disease. The measures are potentially useful in both the clinical encounter and in quality improvement.

But there are certain limitations about QOL measures which we need to be aware of.

They are not a substitute for measures of disease outcomes and may not always be the most appropriate patient centered outcome to assess.

Measures developed for research often cannot be easily used in clinical practice.

One also needs to be aware of the limitations of using numbers to represent a complex concept and above all the subjective nature of these measures as you have already mentioned.

I do not somehow agree with the various applications of measuring QOL . These assessments are generalised means of health care interventions and the means of prioritising funding. But QOL with respect to an individual is a very subjective matter and it should not be generalised.Before/ during a treatment, the patient should be asked to rate his treatment as an individual.

This is solely my opinion and have nothing against the standard procedures that you have so extensively and beautifully covered in your discussion

Hi,

The view that QOL should be defined by the individual rather than the health professional is

supported by several arguments. Firstly, evidence from several studies indicates that patient and doctor assessments of QOL differ and that doctors are unsuccessful in identifying aspects of disease and treatment of importance to patients.

Moreover, the importance of different dimensions of QOL varies among individuals and within individuals over time which means that structured measures may be inaccurate or insensitive.

If QOL measurement is to provide useful and valid assessments for use in clinical practice and outcome research it will need instruments which take account of the uniqueness of QOL to individual patients.
