I am sorry this is lengthy but I need advice... dated 13.11.08 Dear , Following your consultation with me yesterday morning it is fairly obvious that you are not happy with the service or treatment given to you by this Practice, nor are you prepared to accept our diagnosis or advice. I therefore feel that as there has been a complete breakdown in our relationship, it would be better if you registered with another surgery. I am writing today to inform the Health Board of this decision and to ask them to remove your registration details from our Practice list. We have received advice from the Health Board that the majority of practices in Paisley have an open list and should be able to register you without any problem. If you have difficulties finding another surgery please telephone 0141...... and they will advise you. Once the Health Board receive our request you will have 7 days to register elsewhere. Please also be advised that if you have any children under the age of 16, they should always be registered in the same surgery as yourself. Yours sincerely I have applied to register with another practice and have to see the practice nurse on 24 November. Once I have done that I am registered.
