WITH THEIR CHILD WITH MENTAL RETARDATION

Consent Form for Sorting Procedure

I would like to conduct research regarding

parents' ways of coping

with their child diagnosed with mild, mental retardation, between the ages of

4-22 for my

Ph.D. dissertation. You were identified as a potential participant by several

possible

searches. First, you may have been identifies through the review of medical

records of

your child at the Family Center by the Falls. Second, you may have been

identified the

Director at Parma Developmental Center or East Cleveland Early Childhood Center.

Third,

you may have volunteered by responding to a research opportunity article in the

Upside of

Downs Newsletter. Specifically, I am exploring thoughts and behaviors parents

use that are

helpful ways of coping with the daily hassles of parenting their child with

mental

retardation. Information collected for this study will be included in my

dissertation that is

part of the requirement for completing a Ph.D. degree and the results may be

included in

professional presentations that I make as well as in professional publications.

If you decide to participate in this research

study, you will be asked to

take part in a one half hour interview at the Family Center by the Falls, East

Cleveland Early

Childhood Center, Parma Developmental Center, Upside of Downs Support Group or

agreed upon location to arrange a set of 53 cards containing statements

regarding

thoughts and behaviors which would be most helpful to least helpful to you. At a

later

time in this research study after all the data has been analyzed, I may contact

you to ask

further questions about your particular point of view.

Your identity will remain confidential throughout

the duration of the

research study within the limits of the law. All identifying information will

be altered to

protect you privacy. Your participation in this research study is completely

voluntary. If

you choose to participate, you may stop at any time. Although I may serve as a

counselor

for your child and/or family members, please know, that your choosing not to

participate

in this study will not in any way impact the services you and your child or

family members

receive from The Family Center by the Falls, or the Cuyahoga County Board of

Mental

Retardation/Developmental Disabilities (CCBMRDD) or affiliated facilities.

Further, one

would not expect any physical, psychological or social risks from participating

in this

research study. However, if you should become upset over the course of this

research

project and need treatment, please contact Grcevich, MD, Rowane,

MD, or

Jevnikar, LPCC at the Family Center by the Falls or Rick Cirillo, Ph.D.

from the

Cuyahoga County Board of Mental Retardation and Developmental Delays for

references. If

you should need immediate assistance, contact an emergency room nearest you.
