What would determine the most appopriate dose of medcine? TSH level or weight?

Hi, all. I would like to invite all of you here to give any opinions on

it.

I am on the levothyroxine for the first two weeks and waiting for the

added dosage of Levothyroxine by weeks afterwards. I found that some of

Hypothyroidism sufferers have different level of TSH since they have the

same level of antibodies, like TPO. I have a very high level of TSH when

I was first tested without any medication before. Does it mean the

hypothyroidism is more severe than the one who have the lowest level?

And I also wonder if the TSH level which was tested firstly before any

medication would determine the most fit dosage in the process of future

medication or not. The higher TSH level, the more medcine HypoT

sufferers need?

PS. My TSH 42 mIU/l ( 0.35-5.5 ) when firstly tested without medcine,

many times higher than the margin level.

And I also read a book that said that the dosage of L-T4( Synthyroid, or

other brands) is 1.4-1.6 ug/kg (measured weight). Is it true?

That is to say that if one person's weight is 60kg, her/his dosage of

Levothyroxine L-T4 for treatment should be 60*1.4/1.6=84ug/96ug.

Would the dosage be determined by/ related to one's weight??
