Dr. Deborah Nash will be seeing patients on Saturday and Sunday from 8:30am-6:00pm. Our goal is to improve the health and well being of individuals using integrative medicine and, in particular, biomedical treatment options. Specialized testing, not available at traditional labs, will be offered. These tests are designed to determine abnormalities associated with heavy metal toxins, intestinal pathogens, food allergies, immune deficiencies, and more.

Deborah J. Nash, MD, a native of New Carlisle, Ohio, graduated from the Ohio State University College of Medicine in 1978. She completed a three-year family practice residency at Mt. Carmel Medical Center in Columbus, OH and is board certified by the American Academy of Family Medicine. In addition to general family medicine, Dr. Nash has special interests in functional medicine, heavy metal toxicity, and the biomedical approach to treating autism and ADD/ADHD. In addition to her own educational efforts, she has had practitioner training through The Great Plains Laboratory in Lenexa, KS and the Defeat Autism Now! Organization.

She became fascinated with autism spectrum disorders through her experiences with her own children. She has found this personal experience invaluable in diagnosing and treating her patients, not to mention understanding some of the frustrations their parents face in dealing with these problems. Finding underlying causes of autistic and ADHD symptoms and watching those symptoms improve, often dramatically, with treatment has been very exciting and rewarding.

The Great Plains Laboratory, Inc. will provide an experienced Pediatric Phlebotomist that will perform all laboratory procedures for blood collection at the Outreach Clinic. The fee for phlebotomy is $30. All other specimens, including urine and stool will be collected at home. Any other recommended test kits will be given to parents to take home from the clinic. All kits will have a prepaid FedEx envelope to send back to GPL once specimen collection has been completed. Results of testing will be sent to Dr. Nash¢s office within 4-6 weeks. The following panel is being offered at a discounted price: Alternative Autism panel (AAP). This panel is offered at a discounted price of $1,205 for a savings of $213.00 to those using the PROMPT PAY method. Smaller panels are also available.

Please note that we cannot accept BCBS insurance for the IgG food allergy testing. This test must be paid for via cash, check or credit card at the clinic. Shorter panels & additional testing available. Three hour fasting is recommended before the blood draw for the Advanced Cholesterol Panel and the Immune Deficiency Panel. It is encouraged to increase fluid intake at least 24 hours prior to appointment which will help with the blood draw process.

The fee for Dr. Nash¢s initial 40-minute visit is $200.00 and is due at the time of your appointment in the form of a check or cash. Dr. Nash will develop a treatment plan for each child individually according to the results of their testing and can provide follow-up appointments in her private Tipp City office or over the phone to review laboratory test results with patients and their families. The initial laboratory follow-up appointment will usually take 40-60 minutes. This is billed separately and is not included in the outreach consultation fee or laboratory-testing fee. Dr. Nash¢s follow-up fees are billed at $208.00 per hour. We will provide information regarding the physician¢s phone number and office hours at your Outreach Clinic appointment. Please note that Dr. Nash has a cash-only practice and does not accept any forms of

insurance.

The Great Plains Laboratory, Inc is a preferred provider for **Blue Cross Blue Shield PPO insurance**. This is the ONLY insurance that GPL will accept at an outreach clinic for laboratory testing and phlebotomy. This does NOT mean that every BCBS PPO plan will cover any or all of the testing at 100%. We suggest that each individual contact their BCBS insurance company and provide them the insurance CPT codes, which will be provided to you from GPL upon making your appointment. Your insurance company should be able to determine how much coverage you can expect with those test codes. Please be advised that we are unable to accept Medicare, Medicaid, Champus or Tricare plans for payment of any fees and services at the outreach clinic. It is your responsibility to check with your insurance company to find out how much they will reimburse and what your co-pays and deductibles are. The special discounted prices do NOT apply to anyone for whom we are filing insurance. It is meant for those that cannot have their testing covered by insurance. This means that individual insurance prices will apply for each test (as quoted above) for those that want to submit through insurance. If not, the total amount is due at the time of the appointment and can be paid by check or credit card. You will receive a medical receipt with CPT and diagnostic codes that can be submitted to your insurance company (by you) for reimbursement. Many insurance companies will cover at least a portion of laboratory testing

through The Great Plains Laboratory, Inc., but HMO¢s typically will not provide any coverage.

The Great Plains Laboratory, Inc. has enacted a policy requiring each outreach clinic to have a minimum of 15 patients signed up, 14 calendar days before the clinic date. Such policies must be enforced in order to prevent last-minute sign-ups and to make the accurate planning of such events possible. This does not mean that we will not take appointments after the 14-day cutoff, but we ask that patients make their appointments as soon as possible in order for us to ensure that the clinic will take place. In the case of a cancellation, please contact Chasity or Olson at 913-341-8949 at least 72 hours prior to the appointment. Failure to do so could result in a charge to your credit card.
