I have a copy of one we use here in CA from the National Health Federation, which I can snail mail or fax to you, but it basically tells the reader that " the laws in the state of CA say that no child has to be immunized to attend school if he or his parents or guardian advises the school authorities that immunization is contrary to his or her beliefs, regardless of whether such beliefs be religious or otherwise. " Then it has the actual section of the health and safety code. Our statute here says that it is not required for any public or private elementary school. (I assume they mean thru 12th grade by that) And it does say that if the person is suffering from measles (rubella), the person may be temporarily excluded from the school until the governing authority of the private school is satisfied that the disease does not exist. Then the actual form at the bottom says " TO...... In accordance with the provision of Section 9, section 3404 of the health and safety code. I hereby declare as parent of ...., a minor enrolled in your school, I withhold my consent and request that the said minor be exempt from any such vaccination or immunization on the grounds that it is contrary to my beliefs.. Signed...Dated.. Address... Telephone number.... It's that simple. If you'd like me to send it to you just give me your address or fax number. austinsmom@...
