You can look up other Ohio school systems and see what they use. This should be no problem once you have it signed and on record at the school. Typically the schools ask for them to be signed yearly. Good luck. Hope you are well. They absolutely cannot demand a letter from your physician unless you have stated (in writting) that you are declining vaccinations for medical reasons. If it had been a private school it would have been a different story. We are very fortunate to live in Ohio with the philosophical exemption.
