There is S-shaped scoliosis in the thoracolumbar spine convex to the right superiorly and to the left inferiorly 2. T-12-L1- demonstrates a moderate 5mm posterior central protrusion slightly asymmetric to the left. Associated degenerative changes and disc height loss are demonstrated and moderate impression of the anterior thecal sac is noted but there is no foraminal narrowing. 3. L1-2 demonstrates slight disc bulge without central or foramina narrowing. 4. L2-3 is within normal limits without focal protrusion, central or foramina narrowing. 5. L3-4 demonstrates moderate central stenosis due mainly to severe facet degenerative change and ligamenta flavum hypertrophy. Slight 1mm disc bulge is demonstrated. The neural foramina are patent. 6. L4-5 demostrates mild central stenosis due mainly to facet degenerative change and ligamenta flavum hypertrophy. No disc bulge or protrusion identified on the left however, there is moderate right sided 4mm foraminal protrusion with associated moderate right foramina narrowing. 7. L5-S1 demonstrates moderate facet degenerative change and mild disc bulge asymmertric to the right neural foramina with mild right foramina narrowing. Impression: 1. Moderate focal L3-4 and mild L4-5 central stenosis 2. Moderate right L4-5 foraminal narrowing due to foraminal protrusion 3. Guest guest Posted December 9, 2008 Report Share Posted December 9, 2008 Hi Shanna, Please remember that your MRI results are just a small part of how a spinal specialist will determine the best course of treatment for you. A good spinal specialist will use the MRI as a tool and attempt to match the MRI pics to your symptoms; not use the MRI alone do determine what course of treatment you will or will not have suggested to you. The fact that your symptoms have gotten worse and worse over the last few years, is a good indication that some kind of surgery will be suggested. Please, get several other opinions from other spinal specialists, either an orthopedic surgeon that specializes in spines only or a neurosurgeon that specializes in spines only. If surgery is suggested, discuss what type of surgery, get all the information from the doctor and ask if he/she does laminotomies instead of laminectomies with or without micro disectomies on the disc. Moderate left paracentral T12-L1 protrusion > Based on the wording of your MRI reading impressions, the serious parts of your spinal issues are in the L3/L4 and the L4/L5 along with the T12/L1. When stenosis reaches a moderate level with increasing symptoms that become disabling to the patient, surgery is usually suggested to the patient. It doesn't look like, to me, from personal experience, that a 10 week course of PT and or epidural steroid injections will give you much, if any relief in pain or increase your function. Laminotomy is replacing the full laminectomy in surgical decompression. It will be able to keep the spine far more stable and prevent the need for fusions. But only a spinal surgeon can evaluate your specific situation. Be sure that the spinal surgeon you see, also can read and MRI and posts the MRI pics, not only for you, but also he/she can look at as well. Have the spine surgeon show you where your problem is on the MRI and discuss it with him. If he/she does not look at the MRI pics, and does not point out the problems for you, you should find yourself another spine surgeon to see. A good spine specialist will not only explain things to you, but actually show you what is going on with your spine by showing you the MRI pics as well. Good luck and let us know what the spine surgeon says. Your very sweet and knowledgeable! it may not sound bad in words on paper, but I can tell u it feels very painfull lately. yes I am going to see a nurosurgeon next week. if I dont like him, I will find someone else. But that is life, and the experience has taught me that there are many good spine surgeons out there but also many bad ones as well. I've had my share of opinions from both. I too, have to have a doctor that I not only feel comfortable with and caring, but also has the skills and knowledge to be the spine surgeon that he claims to be. I've walked out of more spine surgeon's offices and wanted to prevent them from practicing medicine. If a doctor is offended by my questions, and I don't care what kind of doctor he is, he is history with me. The last time I looked, I am the patient, and the doctor is being hired by me to do a job. Any other person I hire to do any kind of work for me, whether it be medical care, putting on a new roof, or anything else, is going to be grilled by me and answer my questions. If that is a problem, they are not hired. That includes a doctor. I want to be told the truth, I want everything explained, the pros and cons, the risks vs the benefits, show me the MRI pics, explain it to me, etc. If any of that is not done, the doctor is not hired.
