Which PT liability insurance can provide ACORD listing Svetlana Masguotva Instit

Hello Fellow Physical therapists,

For those that haven;t seen my name on the group before, My name is Magda I am a

physical therapist practicing in California.I am writing with an important

question about the liability insurance company that some of you use and the name

of the company that can easily provide the ACORD listing Svetlanas Institute on

the form- what is a requirement before you can be accepted into a trainings.

My insurance company HPSO states that tho only form they can provide is

Insurance Certificate- which is basically the proof of my policy but does not

clearly list the Institute on the form.

I spent over an hour in total trying to explain over the phone what I need but I

wasn't successful receiving the ACORD or any Evidence of Insurance or Memorandum

of Insurance form which would list The Institute.

I really don't know how I can explain what I really need to the customer service

any more, so I ask for your advice.Perhaps the best solution will be for me to

change insurance provider as I plan to continue studying with The Institute.
