I know Zavie will probably email you because he's one that takes Coumadin (warfarin) and another one was Kat from Oz but I don't know if she's actively on the lists anymore. For Zavie, he takes it because of his heart valve replacement but for Kat it was because she had several clots. I THINK, although I can't be sure, that in Kat's case, the estrogen she was taking played a role in the clots forming. If you want to contact her directly, I can find her email address if you want. As for Gleevec being a potential cause, well I can't see how as Gleevec typically inhibits PDGF which to me would suggest that it would have a protective effect but of course I'm no expert and have no real knowledge of how PDGF works anyway. Kinda silly for me to even mention it I guess I found this at http://www.nlm.nih.gov/medlineplus/ency/article/000156.htm " Risks include prolonged sitting, bedrest or immobilization (such as on long plane or car trips), recent surgery or trauma (especially hip, knee or gynecological surgery), fractures, childbirth within the last 6 months and the use of medications such as estrogen and birth control pills. Risks also include a history of polycythemia vera, malignant tumor, and inherited or acquired hypercoagulability (changes in the levels of blood clotting factors making the blood more likely to clot). " Didn't you have knee surgery recently? And didn't you have your INR or PT checked a few years ago which showed that your platelets weren't working well? Perhaps one of these was the cause? I have no experience with checking blood while traveling so I can't help you there but hopefully someone else will but is it safe for you to go on a plane at this point?? I may also have thrown some smaller clot(s) to my lungs. I had a rapid CT test on Friday but haven't gotten the results yet. As good as this test is, it's not really able to pick up smaller clots. I have so far declined an angiogram (the definitive test) because my treatment wouldn't really change anyway. It's an uncomfortable, invasive procedure and I just don't want to do it. Now I'm on heparin and warfarin. I'm doing the heparin injections until the warfarin starts working. Tonight will be my fifth night of heparin injections. I know of at least one other of you other CMLers on warfarin, but I don't recall the reason. I have two issues now. One is the cause, as I don't have any risk factors for DVT. Although I have a desk job, I am up *all* the time, so that's not really a risk. I believe that DVT is listed as a rare side effect of Gleevec. It would be odd for it to happen after 4.5 years on the drug, though, and my vascular specialists and heme don't believe that's the cause. Anyone hear anything about this potential link with Gleevec at all? The other issue is that I am traveling this week for 11 or so days in Alaska. I was going to do the half-marathon for the Leukemia Society's Team in Training, but my docs have said it wouldn't be safe for me to do that, for a number of reasons related to this incident. Hi , I am sorry you have the DVT, which we have already chatted about, and will miss your run....but glad that you will get an Alaska visit in. I have had blood tests a couple times, in Calif, with my doctor Rx from Oregon........I would just make sure that the writing is legible!! Dr. D's nurse always fills it out, and prints his name clearly....and he signs it. Wherever you are I would look in the phone book for a medical lab....or a drop in clinic....give them a call and tell them you want to just drop in for the blood test. The biggest problem I think will be if you want to see the results immediately....which we are so used to........because some places are 'draw stations' only and then send it on to a lab. So, I would ask that question also. But overall, it was not that inconvenient. One thing about the clotting....my local onc said to me just recently that people with leukemia have a greater tendency for clots????? I had a little chest pain with deep breathing and he wanted to rule out a pulmonary embolism for that reason....and I was 'clean'. Have a fun trip. My doc just wrote the order for the blood work, then I took it into a local doctor. They did the blood work right there, then faxed the results to my local doc. I didn't prearrange it, just looked up a local doctor (family practioner) when I got there. There's bound to be a local doctor in Anchorage that you could get in to see for blood work. They should be able to get you in one day while you're there. I know you were looking forward to the race - but there will be other races in your future. Take care of yourself FIRST! I haven't heard anything specifically about Gleevec and the incidence of DVT's, however, I recall hearing that women of child bearing ages on Gleevec and birth control pills containing oestrogen could be at a higher risk. My thought was that it would probably have to do more with the action of the birth control pill's than Gleevec. That being said, I have worked for quite a few years in marketing and sales of the Chronic wounds area of healthcare. My particular area of focus is in venous leg ulcers. Unfortunately 80% of people who develop a DVT may be at a higher risk to develop a VLU later on in life. Have your doctors spoken to you about graduated compression stockings or thoromebotic deterrent stockings (TED)(tighter around the foot than higher up the leg). Too much standing or too much sitting is not good. While you are travelling either by plane or car you should exercise your legs once every hour by pulling your toes towards your knees and then relaxing or pressing down on the ball of your feet while raising the heels. Also while travelling you should drink lots of water, do not cross your legs and wear loose fitting clothing, with the exception of the compression stockings I guess;>) You do exercise a lot running all these marathons so it is weird that you would develop a DVT. I am just wondering if the leg surgery you had last year may have helped you develop this risk for a DVT? Usually any kind of surgery puts a person at a higher risk. I got a written order from my onc for the blood work and went to the local hospital. They drew the blood and faxed the results back to my onc in Denver. I did call the hospital before I left home, just to make sure I dotted the I's and crossed the T's. Best of luck at the marathon! Guest guest Posted June 12, 2005 Report Share Posted June 12, 2005 Hi Jen, Sorry to hear about the DVT, and I have no medical advise to give you, because that's not my area of expertise, but I did find this information on the Novartis site about warfarin and Gleevec: Similarly, certain drugs may force the enzymes to put Gleevec aside, which can result in higher or lower than expected levels of Gleevec in the bloodstream. These effects are known as drug-drug interactions. In particular, Tylenol® (acetaminophen)*, birth control pills (oral contraceptives), blood thinners (especially warfarin, Coumadin®+), herbal products (e.g., St. 's Wort), erythromycin, phenytoin (e.g., Dilantin®‡, and anticonvulsants, are all broken down by the same enzymes that metabolize Gleevec®. Drug- drug interactions may therefore occur with these medications. I'm just wondering whether there could be a drug-drug interaction among your current medications which when combined with a recent surgery as well as the fact that you're a CML patient caused the DVT to occur. Having HAD a drug-drug interaction early in my treatment causes me to be a bit more sensitive to that possibility! I assume your physician is aware of the potential effect of warfarin-Gleevec. I've had 7 injections of heparin so far and my stomach is a mass of purple and red and tiny little hematomas from all of the jabs. And the nurses tell me that I don't look half bad compared to some of the poor folks they see! Thankfully, I am not one to wear those little midriff-baring tops! :-) Unfortunately, my docs can't just write a prescription for the INR testing because they don't have prescribing privileges in Alaska. It would be illegal, and they've all made it clear they will not do that. All they can do is write a letter or something, then I need to find a clinic that will honor the letter and also give me my results immediately - just as you pointed out, . But actually, I'm not technically allowed to " dose myself " with the warfarin, anyway, which I may need to do, so they may not even give me the results since I could make changes in my dosage based on the results they give me! Ack. It would be risky for them, liability and all. I'm hoping , as some of you suggested, to make a few phone calls and see what I can get straightened out before I even go up there. I'm also going to be in Homer and Kodiak, which are a bit more remote than Anchorage, so we'll see how easy that is! Tracey, nope -- I didn't have knee surgery, so that wasn't it. I've gotten my bleeding times and platelets checked a few times in the last few years (it's been over 3 yrs since that episode) and have not had a problem. I did have a big thrombophilia workup, so hopefully that will shed some light on things once I get the results, since I don't have any risk factors (Cheryl - including the pill). Cheryl, thanks for your suggestions about traveling. I did a bunch of research on that too. Actually, I do now have some lovely prescription compression stockings, which the vascular people told me I'm supposed to wear for a year. Wheeee.... (Apparently, once you have a clot, you're at high risk for more.) They also said the surgery was too long ago to be causing a problem now. , you are right that there's a drug interaction between Gleevec and warfarin. What happens, apparently, is that this interaction can make you more likely to bleed. So they're just more careful in the monitoring. When you start looking at all of the drugs and what each one does on its own, nevermind when you're taking multiple drugs, it's scary. Fortunately, I wasn't really taking any meds except for the Gleevec - until now. Barb, I also will not forget that trip to Phoenix. It was sure nice meeting you in person! Unfortunately, I'm apparently one of the last holdouts without a cell phone. My docs has also suggested having the results faxed to them and then they'd just ring me. When we went back to North Carolina a few years ago, Bob had to have blood tests. The dr just wrote a prescription and we took it to the hospital in Statesville. We were able to pick up the results later that day. Perhaps your dr can write that on the prescription that you want a copy of the results, plus faxing one to him. I hope you will be able to make the trip, you have planned so long for this. Hi ..... what's the deal with no privileges in Alaska.....I have never heard of that! I am assuming that you are saying they don't have privileges in any other state? All of the rest of us are saying that we took our oncologist's Rx for blood testing to another state and it was honored....without a problem! Is this because they function as an HMO and can't treat elsewhere? It seems that even if you have a letter and showed up at a clinic, they might have to confirm that it was authentic.....and not you trying to get something on your own.......I suppose they would routinely notarize the letter. Your situation does not seem typical at all.........but also the letter wants to clearly state that they are to give the results to the patient (as Dr. Druker always writes)............. and I don't have a cell phone either!! don't want to be plugged into one. Guest guest Posted June 14, 2005 Report Share Posted June 14, 2005 Hi all, For those of you who have been, no doubt, on the edge of your seats, let me give you a quick update on my little DVT situation. Unfortunately, the results of my CT scan from last Friday were not good, showing clots in both lungs (which explains the symptoms I had last week that made me think I was going to die). The docs told me that they now think I had a large DVT that ruptured, sending a " shower " of clots to my lungs. Fortunately, they apparently were not big enough to actually be fatal. After much discussion, my vascular specialist and I reached a compromise about the treatment while in Alaska for almost 2 weeks. Instead of worrying about warfarin dosing (where you can risk either bleeding out or having another clot), we decided to tide me over with some daily self-injections of heparin. That way, I will not need any monitoring. It's certainly not ideal, but it'll work for now (they actually told me it would not be safe to go on the trip, but I am stubborn - or foolish, or both). :-) Yay. I will spare you the details of how many people read me the riot act for not going to the ER when I actually felt like I was going to die earlier this month, which in retrospect we now know was the clots being thrown to the lungs. Unfortunately, the results of my CT scan from last Friday were not good, showing clots in both lungs (which explains the symptoms I had last week that made me think I was going to die). The docs told me that they now think I had a large DVT that ruptured, sending a " shower " of clots to my lungs. Fortunately, they apparently were not big enough to actually be fatal. Unfortunately, I had a trip to the ER yesterday and apparently may have thrown more clots to my lungs. My doctors had previously told me it was unsafe for me to travel, and this episode yesterday makes it clear to me, as well as them, that it would not be prudent to travel right now.
