H. State ID Card and Questions My daughter with autism turns 17 in April so I am now thinking off all the things I will need to do when she turns 18. I know about legally becoming her guardian. She is receiving services through the Children's Waiver formerly 72D program and just recently has been enrolled in Medicaid because of that. When does the Children's Waiver turn into the Adult Waiver? I didn't think of it until I was reading a post from Ellen about what is required to apply for Medicaid and SSI, but she mentioned an state ID card. How do we do that? Do we have to wait until we become her guardian at 18 to be able to have her get a state ID card? I imagine that they would need a copy of her birth certificate. There is so much to all of this... If you go online to the secretary of state website, you can download the forms to complete ahead of time and see which locations can give you one. Only full service DMVs can do it I think. One thing you want to get is the disability form. If you get this signed by a doctor the card stays valid longer, I'm thinking 10 years, but I'm not sure and it will say disabled on it. I think it's even a reduced rate, but it's been a while. When I called about the disablity form several years ago, the woman told me that you should mention it when you apply for things. It can get you discounts, she could only think of fishing licenses off the top of her head but she said to mention it. I don't think we've ever used it for discounts but it has come in handy to show people. We needed his birth certificate and we needed some form of ID. Again the secretary state website will let you know what you need to bring. I believe this ID is valid until age 21 at which time I need to get a new ID. As for the children's waiver becoming an adult waiver. You need to reapply for the adult waiver any time after you daughter turns 18 but before she turns 22. She can revert at anytime according to PACT. My son was funded for the children's waiver in August. He turned 18 in December and I just applied for the adult waiver last week. It is my understanding that the two work pretty much the same until he leaves school at which time it increases to 3x SSI to help pay for adult workshops which would get paid out of those funds. -- Mar -------------- Original message -------------- From: jillplato@... It is a photo ID that can be used at airports and for medicaid application and throughout their lives. There is a specific form to fill out and as I recall you need to pick up the form and take it home because it will require a physician's signature, verifying that your child has a disability. I am guessing that you can probably find it online through the Secretary of State's website. I cannot remember if you need the birth certificate, but I would think so... I did have his birth certificate with me. He was issued an id. It was easy compared to everything else. The next ID, I took him to the Sec of State--at that point, there was no special ID for those with disabilities. Last year (in February, for his birthday), I again took him to get a new ID and was then informed of the new , special ID and was given a form to be filled out by a physician. Since he was ready, willing and excited to be there, I was not going to make another trip for the new kinda ID and we just got the regular one. He is nonverbal and, while he is soooo cute, it's obvious he is impaired. We were able to go to the head of the line and I don't remember if I had to pay for it--I don't think I did. You can also download the physician form I believe. Next time, we'll get the disability one. I knew he would need an ID to board a plane with me in Feb. He attends a private therapeutic day school that issues no school id's. I didn't know about the special disability ID card, and no one at the Sec. of State's office in Schaumburg mentioned it to me, but they processed Ramon quickly for a regular state ID. They required his birth certificate, his S.S. card, and my DL plus a recent bill verifying my address. It cost $10.
