Guest guest Posted February 10, 2011 Report Share Posted February 10, 2011 Thyroid function test results are absolutely no use to anybody when taking natural thyroid extract and you should not titrate your dose according to what they show. Let us know what the reference range is for each of the tests done Gwenn and we will help with their interpretation. I have never heard that normal levels of free T4 are 4 to 11 - but again, that depends on the reference range being used. When taking natural thyroid extract, it is expected that the TSH is completely suppressed, as it should be, that fT4 is in the upper third of the reference range, and that fT3 is also in the upper third, but more often than not, right at the top of the range where most people feel fine. If this was me, I would be taking the medication that made me feel symptom free. The beauty of NDT is that it quickly tells you whether you are not taking sufficient, and when you are taking too much and you adjust your dose accordingly. Should I cut back to 4 Grains and add in 20mcgs on Lyothyronin to compansate for the loss of T3 ? Can I add that I did not take any med for 36hrs before bloods were drawn. Alk Phos also raised? Hello Gwen, As Sheila has already said, the norm range you quote (4-11) sounds strange.... are you talking about TT4 (total thyroxine) or FT4 (Free Thyroxine) ? Also – how do you know you have a loss of T3? Do you have all the figures at hand by any chance (TSH, FT4 and FT3 and do you know if you have any thyroid antibodies? – this would be important), so that we can see the whole picture, please ? An elevated FT4 (if that is what it is) can have many reasons and without more information it's impossible to comment. As for the Alk Phos (or ALP) .... as far as I know it is not directly linked to the thyroid function, but could be indirectly linked (via D3 levels) .... I have currently the same problem – my ALP is elevated, but my ALK is fine (so no liver damage). When I asked my GP what this could be, he mumbled "Paget's disease?" under his breath and scared the living daylight out of me. However, he then added that depleted Vit D3 can cause ALP elevations, and I should swallow D3 ..... which I am now furiously doing. My Vit D3 is depleted, so I am hoping that this is `it', but I am scared it might not, since I do experience bone pain, which I have always assumed stems from a bone cyst in my hip – but now I am not so sure .... We re-test in 3 months time. So bottom line .... ALP elevations in the presence of normal liver parameters can indicate bone disease – but low Vit D3 levels can also be the culprit. Guest guest Posted February 10, 2011 Report Share Posted February 10, 2011 Hi Gwen, Not taking meds for 36 hours before testing will only have had an effect on the T3 levels- as this has a very short half life. if you take T3, it peaks about 2 hours later, retesting 24hours later and the level will be about half. T4 however, has a long half life of 5-9 days, depending on who you read. So levels will only halve after not taking any for 5-9 days. It's up to you wether you want to reduce your ERFA and replace with T3. there are 9mcg T3 per grain so I would suggest you start there, but may have to increase, depending on how much use you are making of the small thyroid fragments such as T2 and of course if you can convert any of the T4.
