I will call my PAS agent which is PACT but I was hoping someone knows the ins and outs as in the past the PAS agent has been less then helpful. It says the award is contingent on completion of your application process for the Illinois Medicaid program. Medicaid eligibility interferes with medial benefits through the states comprehensive health insurance program (CHIP). Individuals will be terminated from CHIP benefits once enrolled in the Medicaid program. State law prohibits individuals from participation in both CHIP and Medicaid at the same time, so make a choice. An individual may be enrolled in only 1 Medicaid Waiver, has HBWD does that mean he can not have that and these home based services? It also says before services can begin, a Service Plan must be developed by you working with a Service Facilitation agency. Funded Adult HBS may only be rendered by a qualified provider. The Service Facilitator can assist you in meeting this requirement. (is this PACT)? Is says if you have any questions to contact the Independent Service Coordination Agency ISCA staff or HBS service Facilitator. Who do I call first and who will be able to best direct me so that I do not get terminated if nothing is done in 6 months? I know this is a good thing but : I AM SO CONFUSED!
