Guest guest Posted July 29, 2002 Report Share Posted July 29, 2002 This is short story of two patients with full blown AIDS (Advance stages of HIV infection .Mod) admitted with me right now. a) 22 year old educated middle class female got married four years back. 2 years later husband died of AIDS in our hospital only. Now she has come with sudden onset of blindness for last seven days. Inlaws have left her. Even the family members are not very supportive for treatment. Only a very decent father is taking care of his blind otherwise healthy daughter like day to day help, clothing etc. Please advise on how to support the treatment, any employment for her. 26 year old auto rickshaw driver with acute onset seizure and a mass in brain. Most likely Toxoplasma. The treatment is Sulphadiazine and pyrimethamine combination. Sulphadiazine was being distributed for diarrhoea in all our District hospitals till 1992 when it was out of use in rest of the world long back. Now when we need the drug for treatment of Toxolasma the drug has been stopped by the govt supply. It's not available in market also. Although alternative like Clindamycin are available but not a choice drug. Whom to approach for supply of drugs. This patient also cannot afford treatment with antiretroviral due to cost. The issue is same as I raised few months back. It's good to concentrate on prevention and awarenes. In India National AIDS CONTROL ORGANISATION (NACO) has a policy to provide treatment for opportunistic infection, but in real term the Govt. hospitals are not even having the T. Bactrim and T.
