A new service plan was written when he aged out and we started receiving the adult waiver funding. I received this email from our PAS agent today stating that DHS is requesting all the following assessments be done. I was wondering what experiences anyone else has had with transitioning to the adult waiver program. Thanks for any insight you might be able to offer. Tortorelli - Mom to Pat, age 22( Magenis Syndrome) DHS is requiring us to file an updated funding request which includes the following items: 1. Psychological evaluation with IQ score that was completed within the past five years ) 2. Psychiatric evaluation 3. Psychosocial evaluation 4. Copy of Medicaid Card 5. Copy of Social Security Card 6.
