Sue Walter Ensuring that young people with special needs make successful transitions to adulthood requires the involvement of many systems that interface with children and families. Recently, the Illinois Chapter of the American Academy of Pediatrics (ICAAP) and the Illinois Division of Specialized Care for Children (DSCC) have received a 3-year grant from the Health Resources and Services Administration, Maternal and Child Health Bureau to develop transition training programs and resources. In partnership with the Illinois State Board of Education (ISBE), ICAAP and DSCC have developed a school-level questionnaire to gather information about school-based transition services in Illinois. Our goal is to develop a " census " of existing programs in order to tailor resources and training to meet identified needs in Illinois schools and communities. We have included a link to a SurveyMonkey survey at the bottom of this message. We realize that you may or may not be the best person to respond for your school.
