Guest guest Posted January 3, 2012 Report Share Posted January 3, 2012 and all, Further to your question about examples of supports we need to put in place prior to exiting special education at 22 -- Thais is correct that every student will have different support needs and we need to look at each person individually. I've uploaded a couple of document you may wish to print out and ponder (smile). I say 'ponder' because transition planning should be a long and carefully considered process, it doesn't happen in one meeting and certainly requires the entire family, student and school team to figure out what supports are being currently provided and how will the individual maintain those beyond 22? I've uploaded into our Files section, under Transition Planning, a document called Day at a Glance and another called Week at a Glance. Since I've always used calendars to keep myself organized, thought it might make sense to use a similar format with 's planning process. This is the current version we're using, after getting a lot of good input and some constructive tweaking from our NapervilleWorks team, and especially from Cyndi , one of our Transition Vocational Coordinators in Indian Prairie 204 and 's Voc Coordinator as she was exiting. It proved invaluable to us, and I share it here in case it helps others. Sometimes the hardest part about visualizing the future is getting started with even one day. If that's the case for you, start with the Day at a Glance planner & you'll soon see what questions you need answers to and will begin to see what supports currently provided by your school team that you'll need to work on replicating beyond 22. Then it's a matter of expanding that thinking to an entire Week at a Glance. Make sense? For anyone who tries using this, we welcome feedback and suggestions.
