If you reach a point where repeat myotomies and dilatation no longer help because the esophagus is too bent out of shape, (very bad S curve), then they sometimes remove most of the esophagus (esophagectomy). There are different ways to do it depending on how much of the esophagus can be saved and how much of the stomach is in good shape. The most common way for treating achalasia is to convert the stomach into a tube and connect it to what is left of the esophagus in the neck.
