Guest guest Posted February 27, 2012 Report Share Posted February 27, 2012 Hi ,  I see from the time you sent this you are probably already on your way, so good luck with the procdure.  In answer to your question - you know the " drill " that we are all different, so anything I say may have no application to you.  If it is of any help to you, I offer the following anecdote.  Back in the " Dark Ages " of the 1980's when I had dilatations with no anesthesia, typically when the balloon deflated, the pain was gone and it was the " end of the story. " There were actually two times that I left the very next day to go on a vacation, leaving both pain and memory of it behind.  When I had my " post-HM " dilatation in 2006, while asleep (how nice was that!), I was told that a dilatation had been performed. It was worse than any others that I have had lately, but luckily I got it under control after only 2 or 3 minutes. Today I get to try some soft food that I haven't been able to eat since my HM. Soft cooked veggies for tea here I come....no soup tonight! I got a script for this when I first saw my doc but I don't like taking meds unless I really have to. Today I had the worst (and longest) spasm ever, so I rushed to the chemist to get the script filled. It was that bad I nearly couldn't drive to pick my boys up from school. Guest guest Posted February 29, 2012 Report Share Posted February 29, 2012 Hi ,  I keep a little pill box in my pocket (holds about12 nidepine), and never leave home without it. The moment I sense a spasm is about to start, I immediately place a capsule under my tongue, let it dissolve (sometimes once it softens will crack it open with my teeth), and most of the time it will stop the spasm before it even has a chance to develop. While this is a reliable method that I use, it appears that for the majority of us, it either does not help, or the drop in blood pressure that may result from taking it causes dizziness. Other than drinking a carbonated beverage first, if available, it has been the most reliable means of stopping spasms for me in the last 25 years.  If it does help you, there is another side benefit to taking it, in that it relaxes the LES and makes it easier to have meals.  For both spasms and helping eating, I take it on a " when needed " basis only.  For " economic " purposes, nearly 10 years ago, I had it prescribed to me to take 2 capsules a day, 3 times a day. The supply I accumulated that year was so great, I am still working off of it. It's been 17 hours since my worst ever spasm so that's good. Today I can try any food I want, but I am little scared in case I can't eat it. To develop your confidence, just proceed at a very slow pace, even if you feel you should take a sip after each bite. Allow time for the food to go thru before taking your next bite. Today I am having the soft foods I haven't been able to swallow for the last 3 months since before my HM. Today I had a pancake with banana and it was so good. I know it's still a soft food, but it went straight down. I don't want to rush, I want to savor anything I haven't had for months! I could have eaten a small amount of food, but the serving sizes are way too big and it was too late (8pm) in the evening for me to eat. I don't want to fill up my E too much for fear about stretching it. So although I can eat some more solid foods since my dilation, my mind is telling me don't eat!! So I still continue with my tiny meals more often routine! Is that crazy or what? I have a nightly food/drink plan, and when I change it I always wake in the middle of the night with spasms.....always at 4.30am! My plan is to have dinner by 6pm then finish the evening at 8pm with a cup to tea to wash out the E. On the nights I don't have enough for dinner I wake in pain, and if I eat past 8am or don't have my cup of tea I wake in pain....it's that simple! So nighttime socializing doesn't work for me...break the routine and I wake in pain for an hour! A few weeks go by and I get invited out again...I decide I will go and it 'might' by ok this time! Another night in pain and I know it's not OK. Solution...never go out or socialize at night time or pay the price during the early hours and the next day. Guest guest Posted March 9, 2012 Report Share Posted March 9, 2012 Hi ,  I'm not sure that I follow all of your scenarios, but I can see you haven't gotten back to yourself yet on a physcial nor on an emotional level. I am willing to go out on a limb and say to you that your current experiences with spasms is probably temporary, but unable to predict how long it will last. Have you resolved anything as to that strange reaction going on that you were getting that did not end up seeming to be from the nifedipine?  As you know I love to make suggestions, since it appears that " trial and error " is often the way to proceed when it comes to eating and dealing with spasms. I'll address both of them.  Eating out. Eat slower and drink more. Simple advice that may build your confidence back about eating. Also, in your particular case when you go out with your girlfriends, possibly consider having something to eat an hour before to minimize your hunger. As far as those large portions are concerned, can you have them " wrap it up to go " or share a meal with someone if there is not much of a " sharing charge. "  Or just don't worry about the excess. How about ordering a " children's size " portion (that may be pushing it).  Spasms. Many of us have come to terms with it as an inevitable part of our lives and know how to deal with them. We are amongst the fortunate. Back in " the days " I had no idea how to deal with them and I would practically end up in a fetal position from the pain, regardless of where I was. Now " armed " with soda or nifedipine I never worry about it anymore. Its going to happen and I'm always ready. I get a sense that it is going to start and I take my " medicine " before it does.  In your case since you are taking nifedipine, as an experiment try taking one just before you go to sleep at night. As you know, due to necessity, its normally placed under the tongue and allowed to dissolve, for quick action. This time, take it with a drink (for a slower release). Restaurants here will not serve a child's meal unless you are 12 or under...I think that's quite silly! They want you to pay and eat a massive feed! My spasms have been more frequent the last 3 days. About the nifederpine, my doc said he doesn't know of them causing the numbness and tingling, so I will keep taking them and see what happens. The numbness comes and goes, but stopping the pain is best. The trouble with going out is the lateness. I always wait 3 hours after my evening cup of tea before going to bed in the hope that my E is all clear. Last night I couldn't drink a cup of tea at 11pm then wait until 2am to get to bed! With most people (achalasians), we pay attention to what time we finished eating solids before going to sleep. Perhaps this is in the early " post-dilatation " phase for you, but I think you should consider your solid food timing, not your liquid time drinking as to the time frame to wait to go to sleep. Then I think the tea helps wash it down. Perhaps I should try only 2 hours and see if that works. I guess because I only eat small meals all the time I may not need to wait as long either. A person can go several weeks where they have nothing to eat for the 2 hours before going to sleep, then one night they regurgitate. I like to call it an " isolated event " where for some unknown reason an " exception " happens to the " tried and true. "  The person can choose to go longer, or see if it happens again with the 2 hour " limit. "  One problem some of us have, thru no fault of our own, is that we get so focused on having the food when we do, that we might mentally crave it and confuse that for hunger. I know that whenever I go on a diet I feel that I somehow am cheating myself out of something by not having anything " fun " between meals. Once I get accustomed to it, I don't think twice about not eating between meals (or I'll have a fruit in between).  , since you are still adjusting yourself, post-dilatation, without doing anything drastic, you should try to see how you do with the 2 hour time frame. I know I've said it ad-nauseum, but once again; when you have your biscuit with the tea, make sure that before you have the biscuit that you have some drink (tea or otherwise), to made sure you're clear, and that the last bite of the biscuit is followed with a drink. While the problem with the spasms at 4:30 might resolve naturally, at this point you need to do something to turn that " mechanism " off. Maybe changing your eating/drinking time will do it, or the nifedipine " in advance.
