Guest guest Posted April 28, 2012 Report Share Posted April 28, 2012 Hi ,  I had my HM done also in Los Angeles at UCLA back in 2010. I spent along time getting misdiagnosis also. For some people the spasms taper off and eventually you have them only rarely. I had spasms the first month after surgery and have not had one since. I think you are still healing for the first 6 weeks. But, everyone is different and each surgeon does things differently also.  Which kind of HM did you have? Surgery was done at USC (laparoscopic). The spasms come out of the blue, no idea what precipitates them, and now am waking every night with the sharp pains over the area where the camera was inserted (Sharp indigestion pains that last about 10 minutes, again, no pattern to any of these spasms or stomach pains.) Tried eliminating dairy products but that didn't help. I started taking 75 MG Zanta before bed and in the morning, but doesn't seem to stop the nighttime pain. Are you able to eat fairly normally a year post surgery? Do you feel back to normal (pre Achalasia) or do you still have occasional symptoms? Appreciate your sharing your progress, THANKS so much for your reply. I'm starting to wonder if the HM didn't work?! Guest guest Posted April 29, 2012 Report Share Posted April 29, 2012 I was good for about 5 years after my myotomy. Since then I have had rough patches once in awhile with problems swallowing. When these get bad enough, I have had a dilation - 3 times since the HM. Just Friday, I had a botox injection because, although I seem to be swallowing, my esophagus is not emptying right and I wake up coughing. The spasms were horrible at first but have now reduced to maybe two or three times a year. This is not to discourage you at all. I NEVER have been in such misery as I was before my surgery. It was a godsend. I had a GI specialist ask me what I thought my swallowing was after the HM and I said about 85% of normal. All this is new and although a relief that I'll be able to swallow somewhat normally again without chocking and pain, seems as though everyone has different symptoms etc. Such an odd disease that comes out of nowhere with such bizarre painful symptoms. Guest guest Posted April 29, 2012 Report Share Posted April 29, 2012 I never felt like I had GERD in the first place. The pains were something else inside my esophagus. It wasn't any acid coming up, it was the failure of the food to go down and/or some kind of painful contractions. In fact, when I've had esophagrams since then I can see my poor esophagus fluttering even though I don't feel any pain or spasms and they tell me I don't have any peristalsis at all. Please don't be discouraged. Just take it one day at a time. I think everybody is different. It's been a long journey for me. I just had the botox on Friday and had the same damn pain on Saturday morning BUT I feel like I can swallow and lay down flat now. That's something great! I was very concerned about the spasm issue after surgery and talked to my surgeon about what causes that. He told me that they do not know what causes the spasms but from his years of experience that he had learned different techniques to use during surgery that had helped reduce the after effect of spasms by his patients. I am hopeful that he is successful in reducing those for me. I will say from his techniques he found that the normal lengths of cuts in a standard HM surgery did not completely remove the restriction in my esophagus and he ended up cutting more than in a standard HM. I am hoping that because of that it reduces or eliminates the risk of spasms for me in the future. It may be something to talk to your surgeon about or your GI doc. I get the odd twitch if I am stressed but relaxation and a drink usually help. Pre op *any* food that went into my E caused spasms. Now I can tolerate most foods. It is so odd!! My wife has been preparing me nice soups that she has pureed for me and they are a nice change for flavors in food post-op. I have had a pretty good one, no issues with spasm but yesterday nothing would go down so I was panicing once again. Today was a much better day though. I so hope he allows me to start teleworking and loosens some of the restrictions on what I can eat. The last two weeks have been progressively better each day and I am starting to become stir-crazy because I am limited on what I can do physically. I was able to eat out for the first time this weekend and it was a great experience. As far as work goes, I am off work until after Memorial Day Weekend.
