POEM in your case would be a second myotomy and it has been done. I

don't know who would do that. You would have to ask those surgeons that

do POEM if they would. POEM is new so there isn't much data on how well

it works but it looks promising. One thing about POEM when doing a

second myotomy is that because they do it from inside the esophagus

there is little concern about the scar tissue from the lap or Vats or

open previous surgery. So scar tissue from those previous surgeries

would not be reason for an open surgery if POEM is an option.
