At any rate, my son is going to summer camp, and I am to sign a form indicating the following: " In case of accident or other emergency, I hereby grant my permission to have the camp staff authorize medical attention by a physician or admit camper to hospital, as necessary. I do not hold *name of camp* or any of the staff responsible for any accident or injury that should occur. The camp staff will also notify you immediately of any such occurrence. " What can I add to this form to indicate my wishes to not vaccinate, etc.? Is there anything else I should add? Also, I am always concerned about signing something saying I won't hold them responsible for injuries...we're not looking for a lawsuit, but if a staff member was completely negligent or something, I would want to do something about it.
