We get charged through our tax (when we put in our returns). If we are single & earn more than $50,000 we pay an extra 1% medicare levy & if you are a couple/family you can earn up to $100,000 before the levy hits. We get a 30% rebate on private health insurance if we choose to have it & we are exempt from paying the levy. If you don't get private health insurance before you are 30, your private health premium goes up 2% for every year you have not had it (if you are born before a certain year you are excempt though). In Queensland ( & Tasmania & believe) we also pay (through our power bill) an ambulance levy of about $22 a month, so we also do not have to worry about an ambulance bill in an emergency. Every time I read someone from USA talking about insurance, Disability & your medicare I'm literally gobsmacked. I can't believe how hard it is for all of you.
